The

| TERM
LIFE
INSURANCE

Help
your employees protect their families with low-cost life insurance
through the convenience of payroll deduction. Term life insurance
is a cost-effective way to gain security and peace of mind for families.
Eligibility:
All regular full-time, regular part-time or irregular part-time
employees under age 70 are eligible to apply for this employer-sponsored,
employee-paid policy. In addition, any employee's spouse under age
65 may also apply, even if the employee does not participate in
the plan. Unmarried, dependent children may be insured as long as
either of the spouses participate.
Coverage:
Up to $500,000 in life insurance for employees and spouses. For
children, $5,000.
Optional Benefit: Coverage of $100,000 for accidental
death and dismemberment available to employees and spouses.the spouses
participate.
Portability:
Coverage remains in force at the same low rates even if
the employee stops working for your hospital.
Disability
Benefit: Premium payments stop if the insured becomes permanently
or totally disabled before age 60. |

VOLUNTARY
SHORT TERM DISABILITY
INSURANCE

MHA’s
short-term disability program is designed to compliment your current
sick time/earned time policies by allowing you to select a benefit
formula that best fits your needs. Some of the features include:

No minimum participation.
- Covers
only non-job related injuries or illnesses.
- Benefits
begin on the15th day of illness/ accident.
- Benefits
may be payable to 13, 26, or 52 weeks depending on the plan you
select.
- Guaranteed
issue, no medical questions asked.
- Plan
can begin any time during the year you select, with either annual
reopening or reopen at your next benefit reopening date.
- Employees
of MHA member hospitals who purchase the program through the MHA
Insurance Agency,
receive at no additional cost, $10,000 accidental death and dismemberment
insurance.
- Employees
may purchase up to $750.00 per week benefit based on their weekly
salary.
- Minimum
participation by your staff, both at open enrollment and new employee
sign-up.
- Some groups may have the opportunity
to have a weekly benefit of up to
$1050/week.
It
is important to note that under the program, benefits are only payable
for non-work related accidents and illnesses which prevents “double”
dipping.

HIV
INDEMNITY
INSURANCE

The
HIV Indemnity Insurance Plan has been designed to reduce the financial
stress often faced by any individual who tests HIV positive.
More
importantly, it begins paying benefits long before the individual
meets most definitions of disability under employer sponsored disability
plans. Created specifically for health care workers, this innovative
insurance program is available at a reasonable rate. It gives your
employees the security and confidence that they will have some financial
support should they face this crisis.
The
plan features some major advantages:
- Coverage
for all health care employees working at least 4 hours per week
as well as all benefit eligible physicians.
- A
lump-sum benefit paid when an individual, while insured, is first
diagnosed as HIV positive (testing required).

ranging from $10,000 to $250,000 (higher levels of coverage may

As
you are well aware, it has become increasingly difficult, time consuming
and costly to recruit and keep qualified part-time personnel, especially
nurses. To
assist in retaining valuable part-time employees, MHAIA offers a
Voluntary Dental insurance Program with MetLife to those part-time
hospital employees who may not be eligible to participate inn the
group dental plan.*
The
following services are part of the dental program:
- A
dedicated Implementation Leader will be assigned to each hospital.
This project Leader serves as a liaison between you and MetLife
and also takes full "ownership" of ensuring a positive
customer experience during the entire installation of the dental
plan. The Implementation Leader personally meets on-site with
your benefits staff to fully explain the benefits program and
the installation process.
- MetLife
staffs Customer Care Centers that are available via a toll-free
number from 6:00 a.m. to 11:00 p.m. Monday-Friday, and from 6:00
a.m. to 4:00 p.m. on Saturdays. The following information is readily

has a staff of experienced enrollers to conduct on-site employee

enrollment. MetLife has a large number of participating dentists

called MetLink, which enables your staff to:

self-billing statements

premiums electronically

maintains a web site where your employees can download a claim
form, request a directory of participating dentists, and even
receive driving directions to a dentist's office.

Plans, Inc. to provide a self-insured PPO medical plan to MHA members.
The program - now officially named MHA NetworkAdvantage - allows covered
hospital employees and their families access to the Harvard Pilgrim
Health Care provider network through Health Plans, and provides coverage
at significantly lower cost. The MHA NetworkAdvantage plan offers
many unique advantages to MHA members, including self-funding a custom-designed
plan and steerage options that encourage employees to seek services
domestically at specific hospitals and physician groups. |
